All Full-Time and part-time (40+ hours per pay period) employees covered under: 

  • The Management Resolution
  • Plan eligibility is not available to Management Elected, temporary, or seasonal employees

All Full-Time employees (60+ hours per pay period) who have a job title represented by:

  • Criminal Justice Attorneys Association of Ventura County (CJAAVC)
  • Professional Engineers represented by the Ventura Employees Association (VEA)
  • Plan eligibility is not available to temporary or seasonal employees.

Optional Employee-Paid Disability Plans Information

Short-Term Disability

Max Weekly Benefit:
66.67% up to $2,500
Maximum 30 day coverage period

Enrollment Information
If you would like to enroll in this plan, complete the Short-Term Disability section of the enrollment form linked below and return it to your Department’s HR Representative.

First 90 days of your initial eligibility
No additional approval is needed for your enrollment to be processed. Within the first two pay periods after submitting your enrollment form, review your paycheck to verify the deduction. If there are any issues, report them to your HR Representative immediately.

More than 90 days of enrollment eligibility
Once the enrollment form is received and provided to the insurance plan, you will receive an email from MetLife to complete the additional questions needed for them to review and determine your eligibility. If you are approved, your enrollment will be processed, and you will receive a letter from MetLife notifying you of your approval. If approved, review your paycheck to verify the deduction. If there are any issues, report them to your HR Representative immediately.

Long Term Disability Buy Up Plan

(NOTE:  If you fall into the eligibility categories by employee group and regular scheduled hours noted at the top of this page, you are automatically enrolled in the Employer-Paid Long-Term Disability Plan.  The Buy-Up plan provides additional coverage for employees who earn more than the maximum monthly base earnings noted below.  For more information about the Employer Paid LTD-Core plan, see its Certificate of Insurance linked HERE, and review Chapter 5 of the Benefit Plans Handbook.)

Max Monthly Benefit:
66.67% up to $15,000
This plan provides greater income replacement for those with a base salary of more than $12,000 monthly/$144,000 annually (The employer-paid LTD Core Benefit Plan provides a benefit for approved claims of 66.67% of the base salary up to a maximum monthly benefit of $8,000). The 30-day elimination period (waiting period) applies to both the employer-paid LTD Core and voluntary employee-paid LTD Buy-Up Benefit Plans.

Enrollment Information
Complete the Long-Term Disability Section of the enrollment form linked below and return it to your Department’s HR Representative.

First 90 days of your initial eligibility
You can enroll in this plan during the first 90 days of your initial eligibility or beginning the day you receive an increase in pay for which your monthly income initially equals or exceeds $12,000 with No Evidence of Insurability (no health assessment). Review your paycheck within the first two pay periods after your enrollment form was submitted to verify the deduction. If there are any issues, report them to your HR Representative immediately.

More than 90 days of enrollment eligibility
You can also request enrollment in this plan outside of the No Evidence of Insurability periods with a completed health assessment and MetLife’s review and approval of enrollment. Complete the enrollment form linked below if this situation applies to you. Once the request is processed, MetLife will email you with instructions. If you are approved, your enrollment will be processed, and you will receive a letter from MetLife notifying you of your approval. Review your paycheck within the first two pay periods if approved to verify the deduction. If there are any issues, report them to your HR Representative immediately.

If your base salary doesn’t meet or exceed $12,000 per month upon initial eligibility for the plan, you have a second opportunity to enroll without a health assessment within 60 days of your base salary reaching or exceeding $12,000 per month.
